  1. Go to Suite 3146 meeting with Mindy to review GCS agreement ( 38 pages ) and 634 Compensation Package ( 24 pages) from 3:30 to 4:30
  2. Guided by Mindy, meeting with Surrogate Tiffany with her attorney Phil starting from 4:30 , Ensure to bring printed CS agreement ( 38 pages ) and 634 Compensation Package ( 24 pages) for desk-reading ( meaning GC read it by setting up the desk with her attorney)
  3. While GC is reading by the desk with her attorney, open the document GCS agreement ( 38 pages ) and 634 Compensation Package ( 24 pages) working with Mindy on Computer Screen ready to supply the GC information into the blank of these two documents.
  4. Once the documents are ready, guided by Escrow attorney, please let GC read one more time to confirm the personal information is accurate
  5. Escrow attorney, is require dto collect the SSN and Driver’s license for her and her husband as part of this session.
  6. Print the GCS agreement ( 38 pages ) and 634 Compensation Package ( 24 pages) supplied by GC and printed out and then let GC sign
  7. Phi provides the notary once done.
  8. Escrow attorney is required to write the summary of 1059 Screening services. The 1st summary can be guided by Mindy on 1059 QQ group as needed.
  9. Upload the GCS agreement ( 38 pages ) and 634 Compensation Package ( 24 pages) once completed. – Job well-done
  10. If the documents are not fully collected, it is the responsibilities of Escrow attorney to follow-up and explain the terms and collected the final documents as completion of service.

3. Donor 622-846 Summary 档案
4. Donor 1099 筛查文件位于1059群。

以上薪酬$300美元，完成日期为视频拍摄的5个工作日内。如晚于5个工作日，服务薪酬为50%，没有KPI。需要积极主动督促视频制作人员完成视频原创上传
