Dear__________, thank you for attending the video interview today,Iwould like to ask you some questions now about your feelings, thoughts and expectations regarding being a surrogate. Some questions may be sensitive in nature so please bear with me as I ask them. Please realize it is not my intention to distress you but the questions should really be answered in order to understand the reality and seriousness of what we are about to embark upon with your journey as a surrogate.
For first time surrogate:
- may I ask if you have ever been a surrogate before?
- What are your values as a surrogate mother?
- How do you take care of yourself now?
- Can you describe your diet on a typical day when you were last pregnantif there is any?
- How do you feel about the role of being a surrogate for a couple who desperately want to conceive and cannot?
- Having you been pregnant before? I’m sure you realize that very often there will be maternal feelings such as sick, vomiting, even faint arise through the course of a pregnancy, do you have a plan to deal with these feelings should they arise?
- Are you married? If yes, can you describe the relationship between you and your husband?
- If not, do you have alife partner now?And what is the relationship between you and your partner?
- If you are married or having a life partner now, doeshe have any reservations regarding the legal relinquishing of parentage after the birth?
- Should more than oneembryos be implanted and all of them succeed, what are your feelings regarding fetal reduction or abortion if it is the couples’ wish to have no more than onechild, would you even be willing to consider carrying twins?
- What are your views on health during a pregnancy? Will you strive to eat a healthy diet or reach out for support if you need to? should you become overwhelmed with morning sickness or other reasons that may be preventing you from getting the proper nutrition you need for the baby and yourself?
- Do you have a solid support system from your family and/or from your partner to help you face feelings that may be overwhelming?
- Are you willing to work with surrogacy agency, IVF doctors, OBGYN doctors to take all the necessary tests and screening to be qualified and travel to out of state for necessary operations? Are you willing to work under surrogacy supporting staff’s instruction to provide all the data, information that is requested by client?
- How do you see yourself in the bigger picture of the gift you are giving to the couple? Would you expect to see a Christmas card every year and a photo of the baby? Do you have any desire to have future contact with the child as he or she grows into adulthood?
For surrogate with experience, besides the 11 questions listed above , the following questions are extra:
- When was your first and last time as a surrogate mother?
- How were those/that pregnancies(y)? Was there any complications happening? If yes, what is that?
- What kind of support did you receive from your family and/or your partner during those pregnancy ?
- How is the current relationship between you and the client couple?
